Concept notice: Fernwood Gentle Dentistry is a fictional practice and this is a sample policy. A real clinic should have its policy reviewed for compliance with BC's Personal Information Protection Act (PIPA) and its regulatory college's requirements.

What we collect

When you book or contact us we collect your name, phone number, email, preferred contact method, coverage type and anything you choose to tell us in the notes or comfort selections. At your visit we collect the health information needed to treat you safely.

How we use it

  • To arrange and confirm appointments
  • To provide and document dental care
  • To bill your insurer or the Canadian Dental Care Plan, with your consent
  • To remind you of upcoming visits, if you agree

We never sell your information or use it for advertising.

Website forms

Form submissions are sent securely to our clinic inbox through a form-processing service. Please don't send detailed medical information through the website; call us instead.

Cookies and local storage

We don't use tracking or advertising cookies. The site stores two small preferences on your own device: that you've dismissed the cookie notice, and the comfort options you picked, so they can be pre-filled on the booking form. You can clear these anytime in your browser settings. The embedded Google Map may set its own cookies when you interact with it.

How long we keep it

Dental records are kept for the period required by the College of Oral Health Professionals of BC. Website enquiries that don't lead to a visit are deleted after 12 months.

Your rights

You can ask to see or correct your information, or withdraw consent for non-essential uses, by emailing hello@fernwoodgentle.ca. Our privacy officer will reply within 30 days.
